Qualifications

• Graduate of an accredited NP or PA program.
• Current unrestricted APRN or PA license.
• National board certification.
• BLS certification.

Responsibilities

• Provide comprehensive primary care services including preventive, wellness, and acute care visits.
• Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines.
• Develop and oversee longitudinal care plans focused on prevention and chronic disease optimization.
• Manage medically complex patients and address social determinants of health.
• Prescribe medications and therapies within scope of practice.
• Collaborate with nurses, care coordinators, behavioral health providers, and specialists.
• Use population health data and quality metrics to close care gaps.
• Educate and empower patients to actively participate in their care.
