Job summary

Upperline Health is seeking a Nurse Practitioner to provide clinic‑based care in our Hoover, AL market, collaborating with a local Physician team to develop complex plans of care for adults with chronic conditions.

APPs perform diagnostics and treatments in clinic, home, or via telehealth, educate patients on medications, diet, and prevention, and coordinate with a broad interdisciplinary team for value‑based care of at‑risk patients.

Qualifications

• Must have a Master’s degree in nursing.
• Board certification as Adult, Adult‑Geriatric or Family NP.
• Current RN and NP licenses in state.
• BLS certification is required.
• 2+ years NP experience in Primary Care/Internal Medicine.
• Value‑based care experience preferred.
• DEA registration or eligibility.
• Comfort providing care in patients’ homes.
• Experience coaching or supervising clinicians preferred.
• Auto and liability insurance with mileage reimbursement.

Responsibilities

• Develop and implement clinical care plans for adults with chronic conditions.
• Gather history, exam and diagnostics in clinic, telehealth, or home.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Educate patients and families about medications and nutrition.
• Prescribe medications as permitted by state Board of Nursing.
• Maintain medical records per regulations and policy.
• Participate in weekly interdisciplinary team meetings and education.
• Provide telephonic support for escalated triage calls.
