Job summary

Vytalize Health is seeking a Practice Transformation Manager to serve as the primary liaison between the company and assigned provider practices. You will lead on-the-ground implementation of value-based care programs, focusing on practice transformation, population health, and quality performance to improve patient outcomes and contract performance.

You will drive adoption of population health strategies, optimize clinical workflows, and coordinate care across providers, patients, and families.

Qualifications

• Bachelor's degree in Healthcare Administration, Public Health, Nursing, or a related field.
• 3+ years of experience in practice transformation, care coordination, population health, or related healthcare operations.

Responsibilities

• Lead implementation of value-based care models including population health management and practice transformation.
• Collaborate with providers to optimize clinical and administrative workflows using guidelines and quality frameworks.
• Implement and optimize use of EHRs, patient portals, and care management tools.
• Build trust-based relationships with practice admins, providers, and staff to drive collaboration.
