Job summary

Vytalize Health seeks a Practice Transformation Manager to serve as the primary liaison between Vytalize Health and assigned provider practices, leading on-the-ground implementation of value-based care programs.

This role drives practice-level adoption of population health management strategies, clinical workflow improvements, and quality performance initiatives that improve patient outcomes and advance VBC contract performance.

Qualifications

• Bachelor’s degree in a healthcare-related field.
• 3+ years in practice transformation, care coordination, population health, or related operations.
• Experience with physician practices or provider organizations in a value-based care environment.
• Ability to manage multiple practice relationships and deliver measurable results.

Responsibilities

• Lead implementation and deployment of value-based care models and practice transformation.
• Collaborate with providers and staff to optimize clinical and administrative workflows.
• Ensure compliance with regulations and accreditations related to value-based care.
• Develop and implement population health management programs for high-risk patients.
• Facilitate care coordination among healthcare providers, patients, and families.
• Develop performance measurement systems and use data to inform decisions.
