Job summary

Qualifications

Responsibilities

• Provide strategic clinical leadership for the MSSP ACO program and CMS requirements.
• Lead initiatives to improve quality metrics, patient outcomes, and total cost of care.
• Collaborate with clinical staff, administration, and VBC leadership to align operations with performance objectives.
• Develop care models and interventions to improve chronic disease management and reduce readmissions.
• Oversee CMS quality reporting, risk adjustment, and performance improvement activities.
