Job summary

VitalCaring is seeking a Clinical Care Manager (RN) for home health to lead coordination of care and guide clinical decisions, ensuring high-quality patient outcomes. You will act as the clinical quarterback across patients, providers, and teams, delivering timely, compliant care in a fast-paced field setting.

The role requires an active RN license in the practicing state, with BSN preferred and home health experience strongly preferred.

Qualifications

• Active RN license in the state of practice.
• BSN degree preferred; relevant nursing education required.
• Experience in home health and care coordination is preferred.
• Ability to manage multiple priorities with clinical judgment.

Responsibilities

• Lead and coordinate care across the full patient journey.
• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
