Premier Community HealthCare Group seeks a Manager, Value-Based Care to lead strategic initiatives across managed-care and ACO portfolios. You’ll align clinical operations, coding, analytics, and revenue cycle to drive performance, close care gaps, and meet contract targets.

Qualifications

• Strategic leader for value-based care initiatives across multiple portfolios.
• Must understand value‑based care metrics and performance drivers.
• Experience coordinating across clinical operations, coding, analytics and revenue cycle.

Responsibilities

• Lead VBC program strategy with Senior Leadership on growth and sustainability.
• Drive provider engagement, risk-score capture, coding accuracy, and documentation gaps.
• Own VBC dashboards, payer scorecards, ACO data, and risk scores for performance tracking.
• Oversee cross-functional gap-closure and coding huddles to capture high-value opportunities.
• Manage AWV and redetermination workflows, and patient outreach programs.
• Collaborate with Quality and Marketing to improve care-gap closure and patient seen rates.
• Optimize Epic alerts and data workflows to ensure accurate delivered care.
