Job summary

ANGELCARE HOME HEALTH SERVICES LLC is a home health provider seeking a compassionate Physical Therapist to deliver patient-centered care in Vallejo, CA. You will assess, plan, and implement individualized home-based therapy, collaborating with the care team to restore mobility and enhance quality of life.

Ideal candidates hold a CA PT license, a DPT or equivalent, and have strong communication skills with knowledge of Medicare guidelines.

Qualifications

• Valid California Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• Home health or outpatient rehabilitation experience preferred.
• Strong knowledge of Medicare home health guidelines and documentation requirements.
• Excellent communication, interpersonal, and patient education skills.
• Reliable transportation and valid driver's license for home visit travel.
• CPR/BLS certification required or ability to obtain prior to start.

Responsibilities

• Evaluate patients' physical conditions, functional limitations, and rehabilitation needs through comprehensive assessments.
• Develop and implement individualized physical therapy treatment plans to restore mobility and improve quality of life.
• Provide therapeutic exercises, manual therapy, and other evidence-based PT interventions in a home health setting.
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate.
• Educate patients and caregivers on home exercise programs, safety techniques, and injury prevention.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
• Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards.
