Job summary

Saint Francis Hospital, part-time RN in Utilization Management, coordinates patient care and develops care plans. You’ll collaborate with clinicians, monitor progress, and advocate for patients throughout the continuum of care.

The ideal candidate has an accredited nursing degree, CT RN license, and preferred Bachelor’s in Nursing with 5 years in acute care or related settings, plus knowledge of utilization criteria and JCAHO standards; BLS is preferred.

Qualifications

• Graduation from an accredited nursing school.
• Valid RN license in the state of Connecticut.
• Bachelor’s degree in Nursing (preferred).
• Five years of diversified, progressive experience in acute care and/or other settings within the continuum of care (preferred).
• Knowledge and understanding of nationally recognized medical utilization criteria, HCFA/OBRA requirements, State of Connecticut regulatory compliance requirements, and JCAHO accreditation standards (preferred).
• BLS certification (preferred).

Responsibilities

• Coordinate patient care across the continuum.
• Assess patient needs and develop care plans.
• Collaborate with healthcare providers and monitor patient progress.
• Facilitate transitions throughout the continuum of care.
• Advocate for patients’ well-being.
