Applied Home Health Network is seeking compassionate Pediatric LPNs to join our home-based care team serving children in Joliet, IL. Qualifications

• Must be a licensed LPN in Illinois.
• CPR certification required; PALS certification is a strong plus.
• Minimum 3 years of pediatric experience in home health, medical-surgical, or acute care.

Responsibilities

• Conduct comprehensive assessments and monitor the health of pediatric patients.
• Administer medications with pediatric-specific dosing based on age, weight, and surface area.
• Provide hands-on care for children with G-tubes, tracts, ostomies, and ventilator needs.
• Collaborate with physicians, case managers, and therapists to carry out individualized care plans.
• Educate and support families in managing pediatric conditions at home.
• Maintain detailed and accurate documentation using EHR systems.
• Perform pediatric medical-surgical procedures while ensuring a child-friendly, trauma-informed approach.
• Assist in managing pain and providing dialysis support when needed.

We are urgently hiring Pediatric Licensed Practical Nurses (LPNs) who are ready to make a lasting difference in the lives of medically fragile children in a home-based setting. What You’ll Do

• Conduct comprehensive assessments and monitor the health of pediatric patients.

Who We’re Looking For

• Licensed Practical Nurse (LPN) in good standing with the State of Illinois.
• CPR certification required; PALS certification is a strong plus.
• 3+ years of pediatric experience in home health, medical-surgical, or acute care.

Proficient in airway management, tracheostomy care, and infant support.

• Experience with long-term care, medically complex children, or developmental disorders is highly valued.
• Excellent communication skills and the ability to work with families and care teams.
• Tech-savvy with experience using electronic health records (EHRs).
