Job summary

Capitol View Home Health Care is seeking a compassionate Physical Therapist (PT) for PRN home health opportunities throughout Washington, DC. You will conduct in-home evaluations and develop individualized plans of care to help patients stay independent in their own homes.

Qualifications

• Active Washington, DC PT license required.
• Graduate of an accredited physical therapy program.
• Strong clinical assessment and communication skills.
• Dependable transportation for home visits.
• Home Health/OASIS experience preferred.

Responsibilities

• Complete in-home physical therapy evaluations and develop plans of care.
• Provide therapeutic exercise, gait, balance training, transfers and mobility training.
• Assess home safety and identify fall risks.
• Educate patients and caregivers on mobility, safety, and home exercises.
• Monitor patient progress and adjust treatment plans as needed.
• Complete progress assessments and discharge planning.
• Coordinate care with physicians, nurses, therapists and other team members.
• Maintain accurate and timely clinical documentation.

What You'll Do
