• Dates of coverage needed : As soon as credentialed - 10 / 31 with possible ongoing extension

• Scope of Practice : Urgent Care, work related injuries, pre-employment physicals
• Experience : 2+ years of UC and Occ Med experience
• License and certification : Board Certified, KY State License, DEA, BLS, DOT Certification preferred
