Job summary

Peach Tree Healthcare in California is seeking a full-time Nurse Practitioner or Physician Assistant to provide urgent care in our community health center. You will evaluate acute illnesses, injuries, and episodic conditions, document findings in NextGen EMR, and collaborate with physicians to determine optimal care plans.

This role requires California licensure, current BLS, and the ability to prescribe medications per scope of practice.

Qualifications

  • Graduation from an accredited medical school or nursing program with applicable California licensure.
  • Current and unrestricted license to practice in California as NP or PA.
  • Current Basic Life Support (BLS) certification.

Responsibilities

  • Examine and treat acute illnesses and injuries in urgent care.
  • Document encounters, findings, diagnoses, and plans in the EMR.
  • Explain diagnoses and treatment options to patients and families.
  • Order, perform, and interpret diagnostic tests and imaging as appropriate.
  • Perform urgent care procedures (injections, splinting, wound care, etc.).
  • Prescribe medications and vaccinations following guidelines.
  • Coordinate referrals and follow-up with physicians and specialists.
  • Educate patients on diagnosis, medications, self-care, and prevention.
  • Direct and supervise clinical support staff and ensure safety.

We are looking for a full-time Nurse Practitioner or Physician Assistant.

Clinical Duties
  • Examine and evaluate patients presenting with acute illnesses, injuries, and episodic medical conditions to determine appropriate diagnosis and treatment.
  • Document patient encounters, clinical findings, diagnoses, treatment plans, and discharge instructions in the electronic medical record (EMR).
  • Explain procedures, diagnoses, treatment options, follow-up recommendations, and test results to patients and families.
  • Obtain, compile, and record patient medical information, including health history, presenting complaints, physical examination findings, and treatment plans.
  • Order, perform, and interpret diagnostic tests, including point-of-care testing and imaging studies as appropriate, to evaluate acute medical conditions.
  • Perform urgent care procedures including, but not limited to, injections, immunizations, splinting, laceration repair, incision and drainage, wound care, foreign body removal, and infection management.
  • Collaborate with physicians and other healthcare providers regarding complex or high-risk patient presentations.
  • Prescribe or administer medications, treatments, vaccinations, and other therapies necessary to manage acute illnesses and injuries.
  • Prescribe medication dosages, routes, and frequencies based on patient-specific factors and current clinical guidelines.
  • Provide patient education regarding diagnosis, medications, treatment plans, self-care, injury prevention, and follow-up care.
  • Determine appropriate patient disposition, including discharge, referral, transfer to higher levels of care, or emergency department evaluation when clinically indicated.
  • Monitor and reassess patients during their urgent care visit and modify treatment plans as necessary.
  • Refer patients to primary care providers, specialists, emergency departments, or other healthcare services as appropriate.
  • Direct and coordinate activities of medical assistants, nurses, students, and other clinical support staff as assigned.
  • Utilize appropriate personal protective equipment and infection prevention measures to ensure patient and staff safety.
Administrative and Quality Responsibilities
  • Check Peach Tree Healthcare email and NextGen PAQ daily and respond to communications in a timely manner.
  • Participate in peer chart review, quality improvement initiatives, risk management activities, and clinical outcome improvement programs as assigned.
  • Participate in the development, review, and implementation of clinical protocols and standardized procedures related to urgent care services.
  • Maintain complete, accurate, and timely medical records in accordance with organizational standards and regulatory requirements.
Scope of Practice
  • Provide evaluation and treatment of episodic illnesses, minor injuries, and urgent medical conditions according to approved standardized procedures and under physician supervision as required by applicable law.
  • Function under approved standardized procedures in collaboration with supervising physician(s).
  • Practice within the scope of licensure, competency, organizational policies, and applicable state and federal laws.
  • Apply knowledge of urgent care, emergency assessment, and ambulatory medicine principles to the diagnosis and treatment of patients.
  • Order appropriate diagnostic, screening, and follow-up studies related to the patient's presenting condition.
  • Utilize knowledge of pharmacology when prescribing medications and therapies.
  • Provide patient and family education related to acute conditions, treatment plans, and follow-up care.
  • Seek physician consultation for any patient, procedure, or clinical situation that exceeds the provider's level of competence or scope of practice.
  • Coordinate referrals to community resources, specialty care, primary care, behavioral health, or emergency services as appropriate.
Professional Responsibilities
  • Maintain and enhance clinical competency through continuing education, in-services, staff meetings, and educational programs; meet all licensing and certification requirements.
  • Participate in recredentialing activities as required.
  • Comply with all HIPAA, Compliance Program, and organizational policies and procedures.
  • Participate in student intern, preceptor, or job shadow programs as applicable.
  • Provide emergency stabilization and treatment within the capabilities of the urgent care setting when necessary.
  • Perform other related duties as assigned.
  • Abide by all terms and conditions of the employment agreement.
Qualifications
  • Graduation from an accredited medical school and any specialized training appropriate to the position.
  • Current and unrestricted license to practice in the State of California as applicable.
  • Current DEA registration and authority to prescribe medications, if required by position.
  • Current Basic Life Support (BLS) certification required.
Pay range and compensation package

$137,280- $189,280 based on experience
