Qualifications

Responsibilities

• Conducts comprehensive health history and physical assessments for acute problems.
• Develops, manages, and modifies treatment plans.
• Participates in daily huddles and collaborates with the multidisciplinary team.
• Facilitates referrals to community facilities and resources.
• Discharges patients and educates on after-visit instructions.
