Job Description Summary
Full-time Navigator to work at MUSC Health in South Carolina. This position will serve as a patient navigator in our department's FOCUS study; this role links HIV/HCV positive patients to a healthcare provider who can assist in further evaluation and treatment of these diseases. This position also serves as a project manager to help with managing the operations of the project. Job Responsibilities
- Review reports with positive patients. Review health records to determine contact information and any current medical provider. Contact patients with HIV/HCV positive tests and inform of need for follow-up. Provide basic (pre-written) info and refer all additional medical questions to project site (medical) director or follow-up provider. 30%
- Provide project manager assistance for the team including site director and project director. Manages a portfolio of programs and projects. Is the primary point of contact on program and project status, key performance metrics, strategic and near-term plans, and resource management. Connects, coordinates and collaborates among various program and project stakeholders to ensure program and project success. 30%
- Establish relationships with community care providers. Provide patients with option(s) for follow-up and direct contact with these community care providers. Works with community resources and patient to link to first appointment – if patient has primary care, refer to PCP – if no primary care, work with community care partners. 20%
- Continuously communicates with project team to keep screening reports updated. Provide information on patients contacted, patients unable to contact, patients that have made and kept their first follow-up appointment, and reasons no follow-up appropriate. 10%
- Coordinates, maintains and tracks the project request process. Provides regular updates to leadership on program and project status including the identification of risks, challenges and opportunities. 5%
- Performs other duties as assigned. 5%

Minimum Requirements: A bachelor's degree and one year relevant program experience.
