The Medical University of South Carolina Ophthalmology Storm Eye Institute seeks a Patient Navigator to coordinate complex eye care and improve patient adherence and outcomes. The PN will work with cornea and ocular-oncology specialists, facilitate referrals, educate patients and families, and support communication across teams.

  • Care Coordination and Monitoring 35%: review Epic appointments, ensure testing/referrals are complete, coordinate with external/internal partners for procedures, review tests/results with providers, improve clinic efficiency.
  • Communication and Collaboration 30%: serve as primary contact for ophthalmology patients in navigation program, liaise with referring physicians, provide emotional support, connect patients with community services.

Job Description Summary

The Ophthalmology Department is seeking a dedicated and compassionate person to join our multidisciplinary team and become an integral member of the Storm Eye Institute family. This Patient Navigator (PN) position will work directly with the cornea and ocular- oncology subspecialists and maintain a high level of knowledge. The PN will address the need for comprehensive care coordination for highly complex eye patients. The PN will facilitate communication, provide education, and coordinate services to improve patient adherence and health-outcomes. This role aims to increase compliance with medications, treatment regimens, follow up appointments and surgeries while enhancing the patient experience.

Job Description

This position will receive a competitive salary commensurate with experience. This is a full time, on-site position for which candidates must have the ability to maintain flexible hours occasionally including evenings and weekends to accommodate working professional clients.

Job Duties:
  • Clinical Evaluation 35%:
    • Help develop individualized care plans based on assessed needs, including medical management.
    • Teach patients about medications.
    • Support the physician by pre-scribing and live scribing in clinic.
    • Implement communication strategies for persons eye disease/blindness and their caregivers.
    • Facilitate referrals by obtaining clinical information and ensure the information is properly processed.
  • Care Coordination and Monitoring 35%:
    • Review upcoming appointments in Epic to ensure necessary services are arranged before the appointment.
    • Ensure all testing and referrals are complete and necessary items addressed with the provider.
    • Work closely with external partners, such as Glaukos, to help navigate patients through the Crosslinking, Artificial Iris and Oxervate process.
    • Work closely with internal partners, such as the oncology department, to make sure patients have all testing done prior to surgery and that surgery is scheduled efficiently.
    • Review all patient test results, such as imaging and lab work, with providers and execute the care plans discussed.
    • Coordinate with referring physicians' offices to meet the needs of both patients and physicians.
    • Provide comprehensive education on symptoms, disease progression, treatment options, coping strategies, and caregiver support to patients and families, resulting in increased understanding and improved outcomes.
    • Help create clinic efficiencies such as improved clinic flow.
  • Communication and Collaboration 30%:
    • Serve as primary clinical contact for Ophthalmology patients enrolled in patient the navigation program and their caregivers regarding scheduling of testing and appointments.
    • Serve as the liaison between the Storm Eye clinical team and referring physicians.
    • Provide emotional support and counseling tailored to the patient's clinical situation.
    • Communicate with external stakeholders to ensure patients have access to individualized community support services.
