Qualifications

• Experience as a Registered Nurse (RN) preferred.
• Must hold valid RN license.
• Experience in skilled nursing/rehabilitation settings is a plus.

Responsibilities

• Lead a team of nursing professionals in providing skilled nursing care to residents.
• Develop and implement individualized care plans in collaboration with the medical director and nursing staff.
• Ensure compliance with state and federal regulations, as well as company policies and procedures.
• Conduct regular assessments and evaluations of resident care, making recommendations for improvement as needed.
• Provide leadership and guidance to team members, mentoring and coaching to foster professional growth and development.
• Collaborate with other healthcare professionals, including rehabilitation therapists and medical providers, to ensure comprehensive care.
• Participate in quality improvement initiatives, identifying areas for improvement and implementing changes as necessary.
