NexCare WellBridge Senior Living in Dimondale, MI is seeking a Clinical Care Coordinator to oversee an assigned unit within the center’s 24-hour operation and ensure high-quality resident care using the latest technology as part of a strong interdisciplinary team.

The role requires an active Michigan RN license, supervisory experience, and CPR-BLS certification; you will conduct rounds, guide the skilled care process, and ensure timely reports while supporting staff and delivering compassionate,

Qualifications

• Graduate of an accredited school of nursing with active Michigan RN license.
• Minimum 2 years supervisory experience.
• CPR-BLS certification required.
• Analytical skills to develop care plans and address problems.
• Excellent customer service and teamwork.

Responsibilities

• Conduct regular rounds with patients and staff to monitor care.
• Instruct the team on skilled care processes and act as clinical resource.
• Ensure completion of reports (infection control, pharmacy compliance, quality management).
• Supervise nursing team with direction, training, and performance reviews.
• Promote hospitality and excellent customer service to patients, families, and visitors.

How You Will Help People Every Day

• You will conduct regular rounds with patients and staff to monitor performance and care.
• You will instruct the team regarding the skilled care process and function as a resource person for clinical issues
• You will ensure the completion of requested reports, i.e., infection control, pharmacy compliance, quality management, etc.
• You will supervise the nursing team including giving direction & assignments, offering training, and contribute to recognition and evaluations.

Qualifications that you will need to be part of our EPIC team

• Must be a graduate from an accredited school of nursing, with at least 2 years of supervisory experience.
• Must have current RN license in the state of Michigan; current CPR-BLS certification.
• A high level of analytical abilities in order to prepare personalized care plans, implement effective nursing care and rehabilitation, and develop solutions to difficult problems concerning well-being.
