Qualifications

• Experience in long-term care is preferred but not required.
• Strong communication and collaboration with team and families.

Responsibilities

• Assess and evaluate systems that support the delivery of quality resident care
• Resolve resident care issues and family concerns in a professional, timely manner
• Implement and evaluate nursing procedures aligned with unit programs
• Make clinical nursing diagnoses and guide care plans accordingly
• Perform nursing assessments to monitor resident health status
• Develop and implement individualized care plans based on assessments
• Educate residents and families on health conditions and care plans
• Participate in quality assurance activities and ensure compliance
• Maintain accurate documentation per company policy and regulations
• Step in for general nursing duties in emergencies or during staffing shortages
• Use electronic medical records and demonstrate basic computer proficiency

Reliability, trustworthiness, and consistency with attendance are crucial. You must be able to multitask, manage stress, and communicate effectively in a team-focused environment.

Must be a Licensed Nurse (LVN or RN) recognized by the Texas Board of Nursing.
