Job summary

Avardis-Health is seeking a qualified Unit Clerk to join our Norfolk team. The role focuses on accurate medical and administrative recordkeeping, census maintenance, and efficient communication to support nursing staff and residents.

Responsibilities include transcribing orders, recording vital signs, coordinating appointments, and notifying families of changes. The position emphasizes compassionate, detail-oriented service and adherence to care policies in a long-term care setting.

Qualifications

  • High School Diploma or GED is required.
  • Strong organizational, communication, and interpersonal skills.
  • Compassionate, dedicated, and committed to high-quality service.
  • Long Term Care Facility Scheduling experience.
  • Two years' experience in a similar position.

Responsibilities

  • Record medical and administrative information per charting and documentation policies.
  • Maintain the Daily Census Report and submit to Nurse Supervisor/Charge Nurse.
  • Keep an up-to-date roster of residents for the unit.
  • Answer telephone and page calls; deliver messages to residents.
  • Maintain a current listing of emergency phone numbers for the unit.
  • Forward tardy and absentee reports to the Business Office.
  • Record resident identification data on medical records and ID cards.
  • Transcribe physicians' orders to care plans and medication records.
  • Record vital signs as directed.
  • Forward new diet orders or changes to Dining Services.
  • Keep nursing staff informed of residents' appointments.
  • Arrange resident diagnostic and therapeutic service appointments.
  • Notify responsible party when a resident's condition changes.
  • Notify responsible party in accidents or incidents.
  • Fill out accident and incident reports as directed.
  • Admit, transfer, and discharge residents; assist with transportation.
  • Assist in completing admission, transfer, discharge records.
  • Notify Nurse Supervisor if discharge occurs without physician order.
  • Forward completed charts to Medical Records/Health Information.
  • Direct visitors to resident rooms and offices.
  • Embrace company values and daily job function.
  • May be trained for Customer Care Liaison duties.
  • All other duties as assigned.

Major Responsibilities
  1. Record medical and administrative information in accordance with Company established charting and documentation policies and procedures.
  2. Maintain the Daily Census Report and submit to Nurse Supervisor/Charge Nurse.
  3. Maintain an up-to-date roster of residents for your assigned unit.
  4. Answer telephone, page calls; deliver messages to residents, etc., as necessary.
  5. Maintain a current listing of emergency phone numbers for your assigned unit.
  6. Maintain and forward daily tardy and absentee reports to the Business Office.
  7. Record appropriate resident identification data on designated medical records, wristbands, ID cards, etc., as required.
  8. Transcribe physicians' orders to care plans, medication cards, treatment plans, etc., as required.
  9. Record vital signs as directed.
  10. Forward new diet orders and/or diet changes to the Director of Dining Services.
  11. Keep nursing service personnel informed of residents' appointments.
  12. Assist in arranging for and making resident appointments for diagnostic and therapeutic services.
  13. Notify the resident's responsible party when there is a change in the resident's condition as instructed.
  14. Notify, as instructed, the resident's responsible party when the resident is involved in an accident or incident as instructed.
  15. Fill out and complete accident and incident reports as directed.
  16. Admit, transfer, and discharge residents. Assist in arranging for transportation, packing residents' belongings, escorting them to discharge/transfer area, loading, etc., as necessary.
  17. Assist in the completion of necessary medical and administrative records upon the resident's admission, transfer, and/or discharge.
  18. Notify the Nurse Supervisor/Charge Nurse should a resident be discharged without a physician's order.
  19. Forward completed charts of discharged residents to the Medical Records/Health Information Department.
  20. Direct visitors to resident rooms, office areas, etc., as necessary
  21. Embrace Company's core values and incorporate them into one's daily job function.

  23. Conduct oneself with the highest degree of honesty and integrity in every interaction.
  24. Demonstrate a passion for caring as evidenced by interaction with co- workers, residents, families and visitors.
  25. May be trained and assigned to perform the Customer Care Liaison duties as needed.
  26. All other duties as assigned.
Minimum Qualifications
  • High School Diploma or GED equivalent (required).
  • Strong organizational, communication, and interpersonal skills.
  • Compassionate, dedicated, and committed to delivering high-quality service.
  • Long Term Care Facility Scheduling experience
  • Must have two (2) years' experience in similar position.
