Job summary

Phoenix Children's Hospital, Inc. seeks a Part-Time Patient Care Support Assistant to assist in implementing care plans under supervision, and to provide clerical and customer service support for a busy unit.

You will help collect patient data, maintain charts, greet patients, and assist with admissions and chart transfers. Strong teamwork and reliability are essential; BLS certification is required, CNA/LNA preferred.

Qualifications

  • Completion of a course of study leading to a high school diploma or equivalent.
  • Hospital or healthcare experience (preferred).
  • Six months experience with computer data entry or word processing (preferred).
  • Patient care experience (preferred).
  • Current LNA/CNA, or EMT/Paramedic certification OR completed RN clinical skills portion within 2 years of hire (Required).
  • Current BLS Certification for Healthcare Providers (Required).

Responsibilities

  • Collect and record routine patient data to ensure ongoing accuracy of patient charts.
  • Perform basic patient care activities to support comfort and safety of patients.
  • Identify and report unusual findings to healthcare professionals for timely intervention.
  • Maintain inventory levels of stock items, supplies and equipment; photocopy and file documents as requested.
  • Participate in in-service programs, meetings, conferences, and daily care rounds.
  • Assist in maintaining the unit's environment and related tasks as needed.
  • Greet patients and families, answer calls, provide orientation to unit policies and hospital processes.
  • Assemble patient charts for admissions and enter admission orders and demographics.
  • Update and transfer charts to Health Information Management; update chart locator database.
  • Perform miscellaneous job related duties as requested.

Position Summary

This position performs a variety of direct patient care services to assist in the implementation of selected portions of the care plan as directed and supervised by a Clinical Nurse. In addition, this position performs a variety of clerical and customer service support functions for a patient care unit/department to provide for the timely sharing of information, communication and order entry to appropriate sources.

Position Duties
  • As directed and supervised by healthcare professional; collects and records a variety of routine patient data to ensure ongoing accuracy or patient chart. Performs a variety of patient chart maintenance activities to provide for ready availability and accessibility to data.
  • As directed and supervised by healthcare professional, performs a variety of basic patient care activities to provide for optimal comfort and safety of the patient.
  • Identifies and reports unusual findings or observations, clinical symptoms, behaviors, and treatment administered to appropriate healthcare professional to ensure timely and effective intervention.
  • Maintains inventory levels of stock items, supplies, and equipment; photocopies and files documents as requested to provide for efficient unit operations.
  • Participates in a variety of in-service programs, meetings, conferences, and daily care rounds to maintain understanding of care plan changes, issues, and expectations.
  • Performs a variety of activities to provide for ongoing maintenance of the unit's environment; Assist in performance of a variety of related tasks and functions as needed.
  • Greets patients, families, visitors, and care team members to the unit; answers telephone and call lights as appropriate and responds to inquiries both in person and via telephone; Provides orientation to the unit and hospital systems and processes to patients and family members to ensure familiarity with unit's policies and requirements.
  • Assemble patient charts for new admissions and enters admission orders and demographic data as needed to provide for timely and complete patient record for efficient patient care and financial processes.
  • Maintains and updates orders to chart; transfers charts with patient an discharge charts to Health Information Management department. Updates chart locator database to ensure accurate information.
  • Performs miscellaneous job related duties as requested.
