Job summary

Phoenix Children’s Hospital is seeking a Patient Placement Coordinator (RN) to act as the frontline liaison for families and providers, directing admissions, transfers and bed capacity across departments. The role requires 2–3 years ER nursing experience and an active RN license.

You will manage ED referrals, collaborate with admitting, EVS, and case management to ensure timely patient access while upholding Family Centered Care and exceptional service.

Qualifications

  • Two years Clinical RN experience required.
  • Pediatric experience preferred.
  • Current State of Arizona RN license or compact state license.
  • BLS certification required; PALS preferred.

Responsibilities

  • Coordinate transfers and direct admissions; collect intake data from ED, physician offices, clinics and referring facilities.
  • Document acceptance calls and collaborate with MDs to ensure appropriate patient placement.
  • Collaborate with admitting, EVS, and case management to ensure timely admissions.
  • Manage bed board and escalate capacity issues to leadership as needed.
  • Provide exceptional customer service and support family-centered care.

Position Summary

Posting Note: The Patient Placement Coordinators role is the front line of our hospital. You will be the voice, eyes and ears of our families and providers wanting information about the services Phoenix Children’s can offer. This role encompasses having a vast array of knowledge of the internal functions of a hospital; being an educator in communicating to those who are wanting to know more about PCH; as well as using the skills as an RN to direct the patient/physician to the appropriate resources. Incumbent will take calls dealing with highly complex medical cases, and we require a minimum of 2 - 3 years of Emergency Room experience in order to provide the best care for those calling in with complex medical issues. We have FULL TIME and PART TIME positions available for DAY and NIGHT shift, Full Time, AND PART TIME!!! This position REQUIRES an RN LICENSE. This position requires 2 years Emergency Room nursing experience. New graduate candidates should apply to our New Graduate RN opportunities.

This position is accountable for coordination and acceptance of information regarding direct admissions and transfers to Phoenix Children’s. Responsibilities also include coordinating all aspects of patient placement within the facilities, and partnering with nursing and other clinical operation departments to facilitate patient access, throughput, and capacity.

Position Duties
  • Manages requests for all transfers and direct admissions, facilitates provider communication.
    1. Accurately collects the data required for the intake of patients from a variety of locations including: ED, physician office, Phoenix Children’s clinic, referring facilities.
    2. Coordinates all MD to MD acceptance calls; participates in and documents all acceptance calls, with an ongoing focus on quality and customer service.
    3. Collaborates with Patient Placement Representative with determining patient benefits/eligibility.
    4. Manages and accepts all ED referral calls (non-EMS) and communicates incoming referrals to ED.
    5. Demonstrates the ability to triage and prioritize patient information and acuity utilizing critical thinking, clinical decision making, and established process flow to assist in assessing and facilitating patient flow across multiple facilities.
    6. Demonstrates knowledge of human growth and development, assessment, and care of patients of all ages served.
  • Proactively manages all patient placement, throughput and hospital bed capacity.
    1. Manages bed board on an ongoing basis in a timely and accurate manner.
    2. Escalates bed placement/capacity issues to appropriate leadership in a timely manner.
    3. Works with nursing units and leaders to identify best available bed placement at the time of request, given patient needs, staffing resources, level of care requirements, infection control principles, and throughput goals.
    4. Reviews and plans for all scheduled admissions daily.
    5. Collaborates with key departments such as admitting, EVS, case management to ensure timely patient admissions and a positive patient family experience.
  • Acts as a role model for Family Centered Care and Customer Service, demonstrating positive and timely communication with internal and external customers.

    3. Ability to multi-task, including transcribing information into database while speaking with callers.
  • Participates in department specific process improvement.
    1. Assists in tracking and trending barriers in patient flow, transfers or customer service and makes recommendations and assists with solutions.
  • Performs miscellaneous job related duties as requested.
