Job summary

Join Cleveland Clinic Akron General as a Clinical Pharmacist in Unit Based rotations, delivering comprehensive clinical pharmacy services to inpatients. You will perform patient profile reviews, optimize therapy, provide pharmacotherapy consults, and participate in interdisciplinary rounds.

The role blends direct patient care with operational duties, including medication order verification and renal dose monitoring.

Qualifications

• PharmD or BS in Pharmacy from an accredited college of Pharmacy.
• Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocity within 90 days of hire.
• Completion of PGY1 residency, fellowship or relevant post-graduate pharmacy experience.

Responsibilities

• Provide safe, patient-centered, evidence-based medication services.
• Verify medication orders and/or dispense per state and federal regulations.
• Document services completely, accurately and timely.
• Actively contribute to medication safety initiatives.
• Ensure all pharmacy operations comply with national accreditation standards.
• Direct staff and educate students, residents and caregivers.
