Job summary

BCFS Health and Human Services is seeking a TWC Advocate to support youth transitioning out of foster care by providing quality case management and resource coordination. The role requires sound judgment to assist clients in achieving their goals and building lasting community connections.

You will complete intake assessments, coordinate services, support job readiness, and establish partnerships with local employers and training providers. Bilingual in Spanish is preferred where applicable.

Qualifications

• Required – High school diploma or equivalent.
• Preferred – Bachelor’s degree in Health and Human Services, Education, Social Work or Criminal Justice.

Responsibilities

• Complete initial intake assessments on all new clients.
• Assess client needs and coordinate services through local resources.
• Assist clients to learn and prepare for their work environment and monitor their performance during employment.
• Research, identify and solicit commitments from possible sources of employment through community networking.
• Monitor and analyze contract requirements, terms and conditions to ensure compliance with laws, policies and procedures.
• Receive approval for all purchases from Director/ Program Director.
• Turn in receipts and reports for budget and billing in a timely manner.
• Develop training partnerships with local businesses to train clients as productive employees.
• Attend all client staffing events and meetings as required.
• Youth crisis management and leadership goal setting for clients.

Job Summary:

The TWC Advocate is responsible for using sound professional judgementand best practices to ensure quality case management services necessaryto assist youth who are transitioning out of foster care in achieving theirgoals.

Essential Functions:

1. Complete initial intake assessments on all new clients.
2. Assess client needs and coordinate services through local resources.
3. Assist clients to learn and prepare for their work environment and monitor and supervisetheir performance during employment.
4. Research, identify and solicit commitments from possible sources of employment throughcommunity networking.
5. Monitor and analyze contract requirements, special provisions, terms and conditions toensure compliance with appropriate laws, regulations, corporate policies and businessunit procedures.
6. Receive approval for all purchases from Director/ Program Director.
7. Turn in receipts, expense reports, sign in sheets, and other required documentation forbudget and billing in a timely manner.
8. Develop training partnerships between local businesses and local training providers (e.g.community-based organizations or local community colleges) to train client to beproductive employees of the partnering businesses
9. Attend all client staffing’s, department, agency meetings, as required.
10. Youth crisis management.
11. Assist young people in setting and achieving their leadership goals for themselves, family,program, and their community.
12. Make referrals to outside agencies, as necessary, and provide follow-up.
13. Must be able to develop and maintain rapport with community partners, including thelocal workforce, youth and families.
14. Coordinate the use of leadership curriculum with teachers and other staff members andassist with classroom teaching of leadership skills.
15. Perform post-placement and follow up activities such as job coaching, assisting with jobretention, and addressing other opportunities to pursue success.
16. Conduct job site visits and home visits to ensure client gets to the job on time.
17. Assist clients in identifying strengths and weaknesses to maximize job performance,perform task analysis, break down tasks and teaching ways to perform tasks.
18. Seek and establish rapport with local businesses that results in training or hiring of fosteryouth to assist them in becoming self-sufficient, increasing self-esteem, and promotingsustainability and success.
19. Attend conferences and serve on youth councils or task groups.
20. Perform other duties as assigned per grant requirements.

Measurable Deliverables:

1. Submit Monthly reports to Director/Program Director for review.
2. Update required documentation in Salesforce within 48 hours.
3. Document all required data into computer database of prospective employers todemonstrate progress and successes to meet all contract requirements and guidelines.
4. Demonstrate the ability to:
   a. Respond sensitively and competently to the service population’s cultural andsocio-economic characteristics.
   b. Communicate effectively in writing and verbally in English (Spanish whenapplicable)
   c. Work in a fast-paced environment.
   d. Maintain computer literacy required to meet the responsibilities of the position.
   e. Work effectively and without intensive supervision both independently and as amember of a multidisciplinary team.
   f. Travel as needed.
5. Other job duties as assigned per grant requirements.

Requirements:

1. Maintain certificates required for the position including but not limited to First Aid andCPR.
2. Valid Texas Driver’s license with a clear driving record and reliable transportation.
3. Pass a pre-employment drug screen and random drug screens.
4. Provide proof or work eligibility status upon request.
5. Pass a pre-employment and biennial criminal background and motor vehicle check(fingerprints needed if applicant has lived out of state within the past 5 years).
6. Bilingual (Spanish required when applicable)
