Job summary

Personal Touch Home Care in Nassau, NY is seeking a Home Care Registered Nurse (RN) with pediatric experience. The role plans, organizes and directs home care services for adults and geriatric populations; valid NY RN license required.

You will perform assessments, develop plans of care with physicians, administer medications, educate families, coordinate with providers, participate in on-call duties, and ensure documentation accuracy.

Qualifications

• Bachelor's degree in Nursing from an accredited college/university.
• Current active and unrestricted NY nursing license.
• Pediatric care experience in acute or home care.
• Valid driver’s license.
• Current BLS certification.
• Minimum of 1 year acute or home care experience.
• Excellent observation, verbal and written communication skills.
• Basic knowledge of computer operating systems and software applications.

Responsibilities

• Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revisions as patient status changes.
• Develops the plan of care with patient’s physician and families in planning.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing needs.
• Provides healthcare instructions to patient/family per plan.
• Prepares and submits clinical notes and physician updates as required.
