Job summary

Personal Touch Home Care in Bronx, NY is seeking a Registered Nurse experienced in pediatric nursing and adult/geriatric home care. Valid NY RN license and driver’s license are required; BLS certification is essential. Pediatric experience is a plus, and one year of acute or home care is preferred.

The role involves comprehensive assessments, care planning, and collaboration with physicians, families, and other health professionals to ensure high-quality in-home care.

Qualifications

• RN license in NY required.
• Experience with adults/geriatric and special needs populations.
• Pediatric experience is a plus.
• Valid driver’s license.
• BLS certification.
• Minimum 1 year of acute or home care experience.
• Strong documentation and communication skills.

Responsibilities

• Performs comprehensive patient and family assessments to determine home care needs.
• Re-evaluates patient nursing needs and revises the plan of care as status changes.
• Coordinates care with physician; sets goals based on nursing diagnosis.
• Administers medications and treatments as prescribed.
• Counsels patient/family and provides health care instructions.
• Ensures timely discharge planning and documentation per policy.
• Maintains infection control and safety standards.
• Coordinates care among home, community, and specialists.
