Job summary

Hartig Drug Company in Urbandale, IA is seeking a licensed pharmacist to serve as the drug information expert in the pharmacy, ensuring orders are clear, clinically appropriate, and monitored for allergies and drug interactions.

You will verify medications, perform clinical calculations, monitor formulary changes, participate in CMS-required reviews, and support safe delivery of medications while maintaining professional standards.

Qualifications

• Must hold a Pharmacy degree or Doctor of Pharmacy (PharmD) from an accredited college.
• Must have a valid pharmacist license in good standing with the state board of pharmacy.

Responsibilities

• Be the drug information expert in the pharmacy: answer staff and facility questions, ensure orders are clinically appropriate, monitor allergies and duplicate therapy, and release orders from workflow.
• Ensure compliance with all applicable pharmacy laws at all times.
• Check all packaged medications for accuracy of medication, strength, lot and expiration date prior to final delivery.
• Perform clinical calculations (creatinine clearance dosing, IV dosing, warfarin dosing) and support specialty requests.
• Monitor formulary interchanges and promote professional standards among staff.
• Conduct Medication Regimen Reviews for patient changes.
• Ensure deliveries are not released until new orders are processed and medications are prepared.
• Participate in after-hours on-call rotation as needed.
• Perform CMS-required Medication Regimen Reviews as required by law.
• Collaborate effectively as a team member.

Job description

• Be the drug information expert in the pharmacy: answer staff and facility questions, make sure orders are clear and clinically appropriate, release entered orders from work flow doing all necessary interaction, allergy, and duplicate therapy monitoring and ensure all orders are entered correctly.
• Responsible that all pharmacy laws are being followed at all times.
• Check all packaged medications for proper medication, strength, and lot and expiration date information for accuracy before finalizing for delivery.
• Perform clinical calculations as required- creatinine clearance dosing recommendations, IV dosing and monitoring, warfarin dosing, and other specialty requests.
• Monitor and help implement formulary interchanges.
• Promote and maintain professional attitude and performance from all staff members.
• Perform Medication Regimen Reviews for patient acute change or fall requests.
• Be responsible that the deliveries are not sent until all new orders have been processed and meds are being sent.
• Be in the on-call rotation for after-hours coverage
• Perform Centers of Medicare and Medicaid Services (CMS) required Medication regimen as required by law.
• Ability to work cooperatively as a member of a team.
