Job summary

Yale NewHaven Health seeks a Clinical Triage Nurse to join our fast-paced call center. You will triage telephonically, direct patients to appropriate care, and collaborate with diverse clinicians to ensure timely responses and effective patient communication.

Required are CT RN licensure, 3 years of direct patient care, and BLS/ACLS per policy. Strong interpersonal skills, confidentiality, and computer literacy are essential for success in this role.

Qualifications

• RN licensed in Connecticut required.
• 3 years of clinical direct patient care experience required.
• Triage/ICU/ED experience preferred.
• BLS certification and ACLS per departmental policies.
• Graduate of an accredited school of Nursing; Bachelor's or master's in nursing preferred.

Responsibilities

• Performs telephonic triage to determine appropriate level of care using clinical protocols and standing orders.
• Collaborates with a wide range of healthcare clinicians to coordinate care and achieve one-call resolution when possible.
• Communicates with patients, families, and providers to assess needs and guide next steps.
• Provides follow-up on test results, education, escalation, or triage for medical management.
• Adheres to clinical guidelines and documents all activity accurately in the system.
• Coaches and mentors new triage staff and ensures quality standards are met.
