Job summary

Qualifications

• Active RN license (state of practice)
• Experience in triage or clinic-based nursing preferred
• Ability to assess, educate, and coordinate care across diverse patient needs

Responsibilities

• Perform timely and accurate patient assessments across in-person encounters and patient communication channels.
• Deliver comprehensive nursing care including vitals, medications, vaccines, procedures, and safety measures.
• Coordinate follow-up appointments, referrals, and care plans to ensure continuity of care.
