Job summary

Moments Hospice is seeking an experienced triage RN to provide after-hours clinical support remotely. You will assess patient needs, guide care, document in the EMR, and coordinate with physicians and facilities using Telecare protocols.

This role emphasizes independence, patient-centered care, and collaboration with the interdisciplinary team to ensure positive outcomes for patients and families.

Qualifications

• Active RN license without restrictions.
• Hospice care exposure preferred.
• Experience with Home Care Home Base (HCHB) systems.

Responsibilities

• Provide after-hours triage, clinical assessment, and interventions via phone.
• Support patients and families with symptom management and end-of-life care needs.
• Coordinate care with physicians, facilities, and internal teams.
• Dispatch visits and coordinate services with vendors as needed.
• Deliver clear, compassionate education and support to families.
• Document interactions, care plans, and outcomes in the EMR.
• Review team documentation for accuracy and compliance.
• Partner with Telecare staff to ensure positive patient outcomes.

Moments Hospice Triage RN

is a key member of the interdisciplinary team, providing after-hours clinical support to patients and families. In this fully remote role, you will assess needs, provide guidance, and coordinate care using the nursing process and Telecare protocols.This position is ideal for an experienced hospice RN who is confident working independently while delivering compassionate, patient-centered care.

What You’ll Do

Required Qualifications:
