Job summary

STJ St. Joseph's Hospital in Buckhannon, WV is seeking a Registered Nurse to triage incoming patient calls, assess needs, and determine appropriate dispositions.

The nurse will facilitate referrals to WVU Medicine primary and specialty care, and educate patients on immediate care and preventive behaviors. This is an onsite day position requiring an active RN license, BLS within 30 days, and at least 3 years of nursing experience; a BSN is preferred.

Qualifications

• Current Registered Nurse license to practice in the state where services will be provided or a multi-state license via eNLC.
• Basic Life Support (BLS) certification obtained within 30 days of hire date.
• Three years of nursing experience required.

Responsibilities

• Triage callers and assess needs using established criteria and document conversations.
• Provide medical information using established resources and nursing background.
• Identify care gaps and opportunities for intervention.
• Facilitate referrals by scheduling clinic appointments, e-visits, or telehealth with providers.
• Assist consumers in selecting physicians and facilities to maximize access, quality and cost.
• Educate patients on treatment options and preventive care measures.
• Determine nature and urgency of health care needs and disposition via telephone.
• Maintain comprehensive medical records of all conversations.
• Support a team environment with clear communication and proactive thinking.
