Heart to Heart Hospice is seeking a Triage Registered Nurse to provide after-hours telephonic triage for hospice patients and their families, and to coordinate on-call visits.

You will ensure documentation is submitted promptly, manage weekend referrals, conduct admissions and routine visits, educate families, and uphold Medicare regulations and company policies.

Formación

• Current State Registered Nurse License
• Current State Driver’s License, reliable transportation, and provide ongoing valid auto liability insurance
• Two years of experience as an RN (preferred) in a healthcare/clinical setting

Responsabilidades

• Provide telephonic triage to hospice patients and their families during after-hours, weekends, or when the office is closed or at the request of the Executive Director
• Ensure after-hour visits are scheduled with the on-call team in a timely manner
• Ensure documentation is completed and submitted to the site at the end of each shift to ensure continuity of care for the weekday team
• Manage referrals that come in over the weekend and after-hours
• Provide education and support to patients and families/caregivers
• Conduct patient visits as needed (admissions, routine, pronouncements, tuck-ins)
• Promote positive, respectful communication to all staff, referral sources, patients and families
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
