Sawtooth Mountain Clinic in Grand Marais, MN is seeking a Triage Nurse to provide accurate, compassionate phone triage, assess patient needs, and advise or schedule care according to guidelines.

This exempt, full-time role offers $38.00-$42.00/hour, benefits including health, dental, life, and disability insurance, an HSA with employer contribution, vacation and holidays, and a 401k with company match after 6 months.

Qualifications

Responsibilities

• Answer triage phone calls and respond to patient needs.
• Use nursing process and guidelines to advise and schedule care.

• Schedule triaged appointments in the EHR.

• Respond to walk-in triage requests and collaborate with teams.

Job description

The Sawtooth Mountain Clinic (SMC) Triage Nurse provides accurate and compassionate care to patients seeking phone Triage assistance. The Triage Nurse utilizes all resources and guidelines at their disposal to effectively assess, prioritize, advise, and/or schedule patient appointments, or refer calls when necessary to the appropriate medical personnel. The Triage Nurse carries out these and other nursing duties as designated in a manner that is professional and efficient, always prioritizing the care of patients and the needs of the Providers.

Standard Hours: 40 hours/week

Location: Grand Marais, MN

Supervisor: Director of Nursing

Wage: $38.00-$42.00/hour, paid commensurately with educational background and experience.

Job Duties

• Answer SMC Triage phone calls and respond to patient needs as well as calls for SMC Providers
• Accurately and efficiently utilize the nursing process, SMC Triage software, and approved SMC guidelines in order to assist in obtaining the appropriate level of care and/or self-care advice for patients
• Notify patients of lab, x-ray and test results, as directed by SMC Providers
• Ensure appropriate scheduling of triaged patient appointments in EHR
• Execute accurate and thorough documentation and management of electronic desktops according to SMC protocols
• Respond to walk-in triage appointment requests as needed
• Respond to calls or electronic contact from other collaborative teams and agencies such as the SMC Pharmacy team, North Shore Health Care Center as appropriate
• All duties as directed and/or requested by the Health Care Home Team
• All other duties as directed and/or requested by the Sawtooth Mountain Clinic Chief Executive Officer and SMC Providers

Job Qualifications

• Registered Nurse, licensed in MN
• Hospital or Clinic nursing background
• Previous work experience in contact with the public
• Proficiency in EHR systems and with general office technologies
• Organization and administrative skills

Physical Requirements

• Ability to stand for prolonged periods and walk for extensive periods of time.
• Prolonged periods of sitting at a desk and working on a computer.
• Ability to stoop, squat, or bend frequently.
• Manual and finger dexterity and eye-hand coordination to provide nursing care and operating equipment.
• Ability to lift up to 50 pounds.
• Corrected vision and hearing within normal range to observe and communicate with patients and professional staff, in person and over the telephone, and to read printed materials and a computer screen
• Exposure to patient elements including communicable disease and bloodborne pathogens.
