Bluebird Kids Health is seeking a dedicated Triage Nurse to join our CARES Center. The role involves telephonic triage, care coordination, and clinical support to families, reporting to the Operations Manager. The position supports our mission to provide exceptional pediatric care.

Requirements include Florida LPN licensure, BLS, and at least 1 year of clinical experience; bilingual English/Spanish is preferred. This is a part-time shift (Mon-Fri 4pm-8pm) in Florida clinics.

Qualifications

• High school diploma or equivalent.
• BLS Certification.
• Unencumbered Florida LPN licensure.
• Minimum 1 year of clinical experience.
• Strong communication and interpersonal skills.
• Proficiency with electronic health records (EHR).
• Bilingual English and Spanish preferred.
• Pediatric population experience preferred.

Responsibilities

• Pediatric Telephone Triage: provide telephonic triage and guidance or escalate to appropriate level of care.
• Care Coordination: respond to inquiries, coordinate with providers and care teams to manage appointments and education.
• Documentation: maintain accurate documentation in the EHR for continuity of care and compliance.
• Clinical Support: assist during patient visits with medications, point-of-care tests, and intake.
• Compliance and Safety: adhere to HIPAA and healthcare standards for a safe environment.
• Other Duties as Assigned.

Position Description (Shift: M-F 4p-8p)

Bluebird Kids Health is seeking a dedicated and compassionate Triage Nurse to join our dynamic CARES Center team. The CARES Center serves as our enterprise access and remote care hub, delivering essential resources, education, and support to children and their families. As a Triage Nurse, you will play a critical role in ensuring families have timely access to appropriate care through telephonic triage services. Reporting to the Operations Manager, you will collaborate closely with our Practice Operations team to provide seamless care coordination and clinical support, ensuring exceptional healthcare delivery across our pediatric clinics. Your contributions will directly support our mission to provide exceptional care so every child can thrive.

What You’ll Do

• Pediatric Telephone Triage: Provide telephonic triage services, offering clinical guidance or escalation to the appropriate level of care based on established protocols, ensuring timely and accurate access to care.
• Care Coordination: Respond to inbound inquiries, collaborate with providers, staff, and external care teams to manage patient appointments, provide patient education, and address clinical requests via the clinical inbox.
• Documentation: Maintain accurate, timely, and comprehensive documentation of all patient interactions in the electronic health record (EHR) system to ensure continuity of care and regulatory compliance.
• Clinical Support: Work in collaboration with providers to deliver clinical support during patient visits, including medication administration, point-of-care tests, office procedures, immunizations, and clinical intake.
• Compliance and Safety: Ensure adherence to internal policies, HIPAA regulations, and relevant local, state, and federal healthcare standards to guarantee a safe and compliant healthcare environment.

What You’ll Need

• High school diploma or equivalent and relevant nursing education commensurate with nursing licensure
• BLS Certification
• Unencumbered Florida Licensed Practical Nurse (LPN)
• Minimum 1 year of clinical experience
• Strong communication and interpersonal skills with the ability to engage effectively with patients, families, and the healthcare team.
• Excellent organization and time-management skills to handle multiple tasks and priorities efficiently
• Proficiency in using various technology platforms, including electronic health records (EHR)
• Bilingual proficiency in English and Spanish is strongly preferred
• Experience with pediatric populations is strongly preferred
