Job summary

Misericordia Chicago is hiring a Triage Nurse (RN or LPN) to deliver professional nursing care to residents with developmental disabilities on a 24/7 coverage model. The role includes assessments, medication administration, and active IDT participation to maintain consistency with each resident’s ISP.

The position requires Illinois licensure as an RN or LPN, successful orientation, and a commitment to compassionate, person-centered care within the Misericordia Home team in Illinois.

Qualifications

  • Bachelor’s or Associate degree in nursing with Illinois licensure required.
  • Current Illinois RN or LPN license in good standing.
  • Nursing orientation completion (minimum 4 weeks) and IDD experience preferred.

Responsibilities

  • Provide professional, person-centered nursing care to residents on assigned shift.
  • Perform comprehensive nursing assessments and medication administration.
  • Coordinate care and participate in the Interdisciplinary Team (IDT) process.
  • Liaise with families, staff, physicians, and other health professionals to ensure ISP alignment.

Misericordia Heart of Mercy is a home to over 600 children and adults with mild to profound developmental disabilities. Misericordia offers a community of care that maximizes potential for persons with mild to profound developmental disabilities, many of whom are also physically challenged.

Summary

The Triage Nurse (RN) provides professional, person-centered nursing care to residents with intellectual and developmental disabilities (IDD) on an assigned shift, as part of a 24/7 nursing coverage model. The Triage Nurse is responsible for comprehensive nursing assessment, medication administration, care coordination, and active participation in the Interdisciplinary Team (IDT) process. The Triage Nurse serves as a key liaison between residents, families, direct support staff, physicians, and allied health professionals to ensure continuity of care and alignment with each resident’s Individual Service Plan (ISP). The Triage Nurse performs these responsibilities under the supervision of the RN, within the LPN scope of practice as defined by the Illinois Nurse Practice Act.

Responsibilities
  • Participate in RN trainer meetings, discussion, and med cert classes if assigned.
  • Complete SAMA assessments for residents as needed & annual ISPs.
  • Request information from off-campus providers as needed.
  • Attend meetings for observations, new admits, functional decline, or re-admissions to the area (if RN Trainer is unable to attend).
Qualifications
  • Bachelor’s degree in Nursing/official transcripts, or Associate’s degree in Nursing/official transcripts, or completion of an accredited Licensed Practical Nursing program/official transcripts
  • Current Illinois Licensed Registered Nurse (RN) in good standing or Current Illinois Licensed Practical Nurse (LPN) license in good standing
  • Successful completion of Nursing Department orientation and training (minimum: 4 weeks)
  • Good standing with the Illinois Department of Professional Regulation
  • Experience in IDD, developmental disabilities, long-term care, or skilled nursing preferred
  • Proficient in reading, writing, and computer skills
Pay

$39.50-$42 hourly
