Misericordia Heart of Mercy is a home to over 600 children and adults with mild to profound developmental disabilities. Misericordia offers a community of care that maximizes potential for persons with mild to profound developmental disabilities, many of whom are also physically challenged.
The Triage Nurse (RN) provides professional, person-centered nursing care to residents with intellectual and developmental disabilities (IDD) on an assigned shift, as part of a 24/7 nursing coverage model. The Triage Nurse is responsible for comprehensive nursing assessment, medication administration, care coordination, and active participation in the Interdisciplinary Team (IDT) process. The Triage Nurse serves as a key liaison between residents, families, direct support staff, physicians, and allied health professionals to ensure continuity of care and alignment with each resident’s Individual Service Plan (ISP). The Triage Nurse performs these responsibilities under the supervision of the RN, within the LPN scope of practice as defined by the Illinois Nurse Practice Act.
$39.50-$42 hourly
