Job summary

Phaxis is seeking an experienced Licensed Practical Nurse (LPN) to join a busy outpatient medical practice in Huntingdon Valley, PA in a dedicated triage role. This is a full-time, temp-to-perm opportunity offering competitive hourly pay of $22-$28, depending on experience.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Pennsylvania LPN license required.
• Current BLS certification required.
• ACLS certification preferred.
• 3+ years of related clinical experience preferred.
• Previous triage or telephone triage experience strongly preferred.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and telephone skills.
• Ability to prioritize multiple patient needs and responsibilities.
• Comfortable working independently and making sound clinical decisions.
• Strong organizational and problem-solving abilities.
• Experience with EMR systems.
• Basic proficiency with Microsoft Office applications.

Responsibilities

• Conduct telephone triage and assess patients’ symptoms and concerns.
• Obtain accurate medical histories and relevant clinical information.
• Determine appropriate next steps using established clinical protocols.
• Communicate patient concerns and changes in condition to physicians and other healthcare professionals.
• Respond to calls regarding test results, medication refills, post-operative instructions, and general medical questions.
• Provide clear patient education regarding medications, treatments, and follow-up care.
• Handle complex prior authorizations and medical clearance requests.
• Initiate appropriate diagnostic or therapeutic measures according to established protocols.
• Coordinate patient needs with providers and other departments.
• Assist with resolving patient-care concerns efficiently and professionally.
• Maintain accurate and timely documentation within the EMR.
• Ensure compliance with HIPAA and organizational policies.
• Provide compassionate and professional support to patients and families.
• Assist with additional clinical responsibilities as needed.
