DIGITAL HEALTH INFORMATION LLC is hiring a Licensed Practical Nurse (LPN) to join their clinical care navigator team in Huntingdon Valley, PA. The LPN will engage in remote patient care, monitor vitals, assist in telehealth appointments, and support care navigators with patient intakes. Candidates must have a valid nursing license in Pennsylvania and demonstrate excellent communication and customer service skills. The role offers a proposed schedule of Monday to Friday, 9:00 AM to 5:00 PM, with compensation between $25.00 - $28.00 per hour based on experience.

Qualifications

• Graduate from an accredited school or program of practical nursing and hold a current license from the State of PA.
• Experience with both front office (patient interaction) and back office (support staff) activities.
• Familiarity with EHR systems (eClinical Works preferred).

About the Company

Digital Health Information is a new and innovative company that provides dedicated care to the geriatric community, specifically to patients residing in their own home. We are an independently owned LTC pharmacy (not open to the public), telehealth physician practice, and remote patient care facility. As a new ecosystem we are working to expand our operations nationwide, one state at a time, and currently operate in Pennsylvania and New Jersey localities.

OVERVIEW

The Licensed Practical Nurse (LPN) will be part of our clinical care navigator team. They will be responsible for day-to-day front and back-office activities. Such activities include regular correspondence between our staffed physicians, our patient population, and our care navigation team. Our LPNs provide remote patient care by monitoring vitals and keeping regular contact with their patients on a weekly basis. They assist the physician in conducting telehealth appointments and documenting outcomes to ensure patient success. Additionally, they collaborate with our Care Navigators to perform new patient intakes, home assessments, and serve as a clinical resource for the Care Navigator Team. The LPN will also support our on-call queue to provide additional patient assistance.

RESPONSIBILITIES

• Perform patient outreach via phone.
• Assisting the physician in conducting telehealth appointments with patients.
• Monitor patient vitals.
• Following up with patients on the progress of their care on a weekly basis.
• Outreach to patients in times of concern.
• Support Care Navigators with patient intakes and home assessments.
• Schedule device deployment and installations for patients based on patient diagnosis.
• Follow all applicable government regulations, including HIPAA.
• Other duties as assigned; Job duties may vary depending on business needs.

QUALIFICATIONS

• High School diploma or equivalent required
• Graduate from an accredited school or program of practical nursing and hold a current license from the State of PA (additional hold license for any NLC states- preferred)
• Basic Life Support provider
• Current CPR certification (or within 90 days of hire)
• Experience with both front office (patient interaction) and back office (support staff) activities.
• Knowledge of professional LPN concepts and principles
• Familiarity with Electronic Health Record (EHR) systems (eClinical Works preferred)
• Strong attention to detail and accuracy

• Ability to work effectively in a fast-paced environment

• Valid Driver’s license and means of transportation

Proposed Schedule

Mon- Fri 9:00am-5:00pm (40 hours per week)

Compensation & Benefits

$25.00 - $28.00 per hour (negotiable based on experience)
