Job summary

Optimal Care is seeking a Triage Coordinator to manage after-hours calls, orders, and coordination with on-call providers, patients, facilities, and families. The role requires initiative, problem-solving, and strong communication in a geriatric-focused practice.

You will handle after-hours planning, coordinate care plans, and maintain confidentiality while supporting a team dedicated to compassionate, high-quality care. This position is hybrid with in-office and remote shifts.

Qualifications

  • High School Diploma or GED required.
  • Minimum one year of experience in medical field, medical office, or related position.
  • Proficient in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
  • Knowledge of medical terminology.
  • Excellent verbal and written communication skills.
  • Ability to operate computer equipment, fax and photocopy machines.

Responsibilities

  • Coordinates after-hours calls, orders, and communication with on-call provider, patients, facilities, and families.
  • Communicates significant findings or changes to primary care providers and care team.
  • Manages after-hours planning, coordination, and evaluation of plan of care for patients and families.

  • Maintains confidentiality of patient and employee information.
  • Responds to after-hours inquiries and changes in patient condition.
  • Provides guidance to agency staff, healthcare professionals, patients, and families.

Key Responsibilities

The Triage Coordinator is responsible for the coordination of all after hours calls, after hours orders, and after-hours communication with on-call provider, patients, facilities, and families for Physician Services. Responsibilities are completed with limited supervision and after hours. This position requires initiative, creative problem resolution, time management, excellent phone and electronic communication skills, service excellence and a desire to work within a practice that centers around and with a geriatric population.

In this role you will be responsible for:

  • Communicates significant findings, problems, changes in condition or environment to the primary care providers and other personnel involved with patient care
  • Performs well under stress and in emergency situations
  • Assumes primary responsibility for “after hours” including planning, coordinating, implementing, and assisting with the evaluation of the plan of care for patients, families and facilities
  • Develops and maintains positive working relationships with others. Helps others when needed without necessarily being asked to do so
  • Maintains confidentiality of patient and employee information. Shares information only in accordance with policy
  • Answer telephone calls after hours and responds to any concern or change in patient condition
  • Provides support and direction to agency staff, health care professionals patients, and families related to health care concerns

Required Qualifications

  • High School Diploma or GED
  • Minimum one (1) year of experience in medical field, medical office experience or related position
  • Ability to operate computer equipment, Fax and photocopy machines necessary
  • Proficient in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint
  • Knowledge of medical terminology
  • Excellent verbal and written communication skills

Desired Qualifications

  • CNA or MA certification preferred
  • Experience with geriatric population and/or caregivers preferred
  • Familiarity with Ethizo EMR preferred

Location

  • Home Office: One of the following
    • Jackson, MI
    • Kalamazoo, MI
    • Grand Rapids, MI
    • Saginaw, MI

Schedule Overview

  • Four shifts per week with an every-other weekend rotation
  • The schedule follows this pattern:
  • Week 1: Work three weekdays + the weekend
  • Week 2: Work three weekdays (no weekend)
  • This rotation repeats

    Daily Schedule

    • In the office: 11:00 a.m. – 4:00 p.m. or 11:00 a.m. – 5:00 p.m.
    • Meal break: 4:00 p.m. – 5:00 p.m. or 5:00 p.m. – 6:00 p.m.
    • Work remotely from home: 5:00 p.m. – 10:00 p.m. or 6:00 p.m. – 10:00 p.m.

    This schedule provides a combination of in-office collaboration during the day and remote work in the evening, along with every other weekend coverage.

Pay Range

$18 - $22 USD
