Job summary

Humankind Foster Care is seeking a full-time Treatment Foster Care Case Manager in Richmond, VA. You will assess foster children’s needs, develop plans, coordinate home visits, and collaborate with courts, families, and service providers to deliver supportive services.

The role requires a bachelor’s degree with child/family casework experience or a social work degree, valid driver’s license, and ability to travel up to 50% within a 100‑mile radius.

• Bachelor’s degree required; two years of casework with children/families or social work degree with one year experience
• Valid driver’s license and minimum auto insurance
• Position may require up to 50% driving within 100 mile radius

Responsibilities

• Complete and maintain documentation (monthly notes, reports, 14/30/45 day plans, histories, quarterly/annual reports)
• Assess foster children needs and implement objectives and services
• Conduct home/field visits multiple times monthly for each child
• Attend/coordinate meetings (FAPT, Court, FPM, Treatment Teams, VEMAT)
• Supervise visitations and arrange transportation and referrals
• Participate in trainings and audits
• Support and train foster families

Success In This Role Looks Like:

• Complete and maintain thorough documentation including but not limited to: Monthly Notes and Reports, 14 Day Plans, 30 Day Plans, 45 Day Plans, Social Histories, Quarterly Reports, and Annual Reports.
• Assess the needs of foster children, and implement tangible objectives, strategies and services to address all identified needs.
• Completion of home and field visits multiple times a month for each child.
• Attend or coordinate meetings on behalf of foster children and foster families including; FAPT, Court, FPM meetings, Treatment Teams, and VEMAT meetings.
• Supervise visitations between foster children and their birth families, provide transportation for identified services, and make service referrals to various agencies.
• Participation in ongoing trainings required by LCPA standards, HumanKind, and supervisor.
• Participation in all audits, to include internal and external conduct by LCPA Licensing Inspector.
• Use Placement/Matching protocol in the event assistant is needed for placement by the Program Manager.
• Provide ongoing support and training to foster families.
• TFC Case Manager > Quality Assurance Specialist > Quality Assurance Manager

• Comply with LCPA standards, HumanKind and HK TFC policies and procedures, and remain up to date on any changes.

Requirements for Consideration:

• A baccalaureate degree in any field plus two years of experience in providing casework services to children and families;
  • OR a baccalaureate degree in social work or a field related to social work including, but not limited to sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families;
  • OR a doctorate or master’s degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling with a student placement in casework services to children and families or one year of experience in providing casework.
• Valid driver’s license, acceptable driving recor, and m inimum personal auto insurance coverage.
• Position requires up to 50% driving time within a 100 mile radius.
• Complete, pass and maintain all required Background checks and drug test .
• Must be able to response to on-call emergencies within an approved amount of time as determined by Program Manager.
