Eating Recovery Center Pathlight in Denver is seeking a Treatment Coordinator to be the initial contact for patients and families, coordinating admissions and transitions across care levels. You will support treatment teams with surveys and assessments, and ensure a positive patient experience.

The role requires a high school diploma or GED, with a Bachelor’s degree preferred. The schedule is Monday through Friday, with 7:30am–4pm hours, and a compensation range of $20.43–$25 per hour.

Responsibilities

  • Admissions and Patient Transitions: greet and assist patients/families; identify referrals for varying levels of care and coordinate with treatment team.
  • Coordinate discharge and transfers to other treatment facilities.
  • Assist with patient admissions in medical records system; register patients in the portal; manage forms and schedules.
  • Administrative duties: greet visitors, obtain signed confidentiality statements, issue badges, route calls; maintain supplies and tidy spaces.
  • Coordinate transportation and submit daily attendance from sign-in/out logs or census; assist with outside medical appointments as directed.

The Treatment Coordinator serves as the initial point of contact for patients, families, and the public, ensuring they are greeted and assisted in a professional and supportive manner. This role plays a critical part in supporting a positive patient experience by coordinating patient admissions and transitions across levels of care.

Additionally, the Treatment Coordinator provides administrative support to treatment teams and ensures the timely distribution and completion of treatment plan assessments and patient engagement surveys

Essential duties and responsibilities:

  • Admissions and Patient Transitions:
  • Sends welcome emails or other information to patients/families
  • Identifies referrals for varying levels of care/treatment and coordinate withtreatment team and providers to secure these services for patients
  • Coordinates discharge and/or transfers to other treatment facilities
  • Assisting with patient admissions: admitting the patient in medical records system,adding treatment team members, helping patientsregister in the portal, scanningadmission documents, issuing admission day schedules, uploading appropriateforms, folder prep, print schedules
  • Administrative Duties:
  • Greeting/assisting visitors entering the facility
  • Obtaining signed confidentiality statements, issues vendor badges (ifneeded) and informs appropriate staff of arrival
  • Answers phones and directs calls to appropriate person
  • Maintaining supply of patient/family binders and discharge packets
  • Ensuring staff breakrooms are stocked, lobby and common areas are tidy
  • Overseeing of incoming and outgoing mail/packages
  • Coordinating transportation
  • Submits daily attendance gathered from sign in/out logs or midnight census
  • Program Support:
  • Coordinates outside medical appointments for patients as directed by medical or nursing staff

Qualifications:

The compensation range for this position is based upon candidate experience and market expectations.

$20.43 - $25 USD
