Position Summary
The Treatment Coordinator is a licensed behavioral health professional responsible for providing structured, face‑to‑face clinical sessions and coordinating all aspects of client treatment, including referrals, collaboration, and care transitions. The role also conducts weekly audits of treatment and documentation to ensure compliance with OMHC standards and COMAR requirements.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Clinical Evaluation & Assessment
- Complete or ensure completion of an initial face‑to‑face diagnostic evaluation for each assigned client within required timelines (no later than the second visit).
- Document presenting problem(s), psychosocial and family history, mental status, risk factors, strengths, functional impairments, and current needs.
- Maintain collaboration with a client’s primary care provider or other somatic providers when clinically indicated.
- Update evaluative information as clinical status changes.
- Treatment Planning & Documentation
- Develop an Initial Individual Treatment Plan (ITP) no later than the fifth visit.
- Collaborate with the client and family (when applicable) to draft ITPs.
- Ensure each ITP includes:
- Current diagnosis
- Presenting needs, strengths, recovery goals, and treatment expectations
- Specific services and interventions (modality, frequency, duration, responsible staff)
- How services will help manage symptoms and support recovery
- Short‑term and long‑term goals written in behavioral, measurable terms with target dates
- Referrals and collaboration with other providers or agencies (e.g., PRP, housing, vocational, substance use services, schools, social services, juvenile services, somatic care)
- Conduct monthly treatment plan reviews with each client, revising goals and interventions as needed.
- Ensure documentation is accurate, thorough, and completed within required timeframes.
- Direct Client Intervention (15 Hours per Week)
- Maintain a caseload of approximately 20 clients.
- See four clients each morning for 45‑minute sessions, ensuring each client is seen weekly unless cancellations or clinical decisions alter frequency.
- During sessions: review progress, address barriers to engagement, provide motivational enhancement and skill‑building, educate about risks of AMA, and begin or support safety planning.
- Address client concerns and complaints related to services and escalate as appropriate.
- Treatment Coordination & Referral Management (25 Hours per Week)
- Coordinate all services per the ITP, ensuring delivery matches authorization and documentation.
- Communicate regularly with psychiatrists, nurse practitioners, therapists, PRP staff, case managers, and other providers.
- Issue and track referrals to internal and external resources such as PRP services, substance use treatment, housing, vocational/educational services, primary care, social services, and other community supports.
- Monitor and document somatic status information in the medical record per OMHC regulations.
- Identify and troubleshoot barriers (transportation, insurance, communication) and collaborate with staff and agencies to keep care on track.
- Participate actively in efforts to retain clients in treatment as clinically necessary while planning for step‑down or transition when appropriate.
- Weekly Treatment & Team Audits (Quality Oversight)
- Conduct structured weekly audits of treatment for each client, assessing alignment with ITP, service frequency and duration, goal progress, and discharge/transition planning.
- Review documentation of treatment team members for completeness, timeliness, clinical appropriateness, and regulatory compliance.
- Identify non‑alignment or non‑compliance and implement corrective actions.
- Update treatment plans as needed based on audit findings and document activities in client records and quality management tools.
- Intake, Orientation & Discharge
- Participate in or conduct intake evaluations for clients added to the caseload.
- Participate in discharge planning and continuity‑of‑care processes, ensuring a documented discharge summary with required details.
- Promote smooth transitions to higher or lower levels of care or to other providers when medically necessary.
- Team Leadership & Staff Support
- Serve as Treatment Team Lead for assigned clients.
- Provide day‑to‑day clinical direction, coaching, and feedback to team members.
- Address performance issues related to documentation, timeliness, or ITP compliance.
- Recognize staff accomplishments and promote a collaborative, recovery‑oriented, culturally responsive team culture.
- Meetings & Supervision
- Participate in weekly clinical supervision when required by license and agency policy.
- Meet weekly with the Operations Manager, Human Resources representative, and Finance representative to review caseload, staffing, training, billing, and payer requirements.
- Attend all required team meetings, case conferences, and agency trainings.

MINIMUM QUALIFICATIONS
Licensure – State of Maryland (one or more of the following):
- Licensed Clinical Professional Counselor (LCPC)
- Licensed Graduate Professional Counselor (LGPC)
- Licensed Certified Social Worker (LCSW)
- Licensed Certified Social Worker–Clinical (LCSW‑C)
- Licensed Master Social Worker (LMSW)
- Licensed Bachelor Social Worker (LBSW)
- Licensed Clinical Alcohol and Drug Counselor (LCADC)
- Licensed Graduate Alcohol and Drug Counselor (LGADC)
- Certified Associate Counselor–Alcohol and Drug (CAC‑AD)
- Certified Supervised Counselor–Alcohol and Drug (CSC‑AD)
- Advanced Practice Registered Nurse – Psychiatric Mental Health (APRN‑PMH)
- Certified Registered Nurse Practitioner – Psychiatric Mental Health (CRNP‑PMH)
- Licensed Psychologist
Candidates must hold an active, unencumbered Maryland license or certification in one of the above categories.
EXPERIENCE & SKILLS
- Prior experience in outpatient mental health, community mental health, or related behavioral health settings strongly preferred.
- Knowledge of recovery‑oriented, person‑centered, and trauma‑informed care principles.
- Strong skills in clinical assessment, treatment planning, and documentation.
- Demonstrated ability to coordinate care across multiple providers and systems.
- Ability to lead a multidisciplinary treatment team, provide constructive feedback, and support quality improvement.
- Strong organizational skills and ability to manage time, competing priorities, and regulatory deadlines.
- Comfort with electronic medical records and basic computer applications.
- Reliable transportation for occasional travel to community sites where services are rendered.
