Job summary

Four Winds Hospital in Saratoga Springs, NY seeks a full-time Treatment Coordinator to join an interdisciplinary team on adolescent units. The role handles case coordination, discharge planning, and family engagement, working onsite from our Saratoga Campus.

Qualifications

• Must hold a NYS Limited Permit as a Mental Health Practitioner.
• NY License as LMHC, LMSW or LCSW preferred.
• Two years post-master’s clinical experience in individual, group and family therapies.
• Strong organizational, interpersonal, and communication skills.
• Experience with adolescent populations preferred.

We are seeking applicants for a full time Treatment Coordinator to work within an interdisciplinary treatment team on the adolescent units.

The job is in-person onsite at our Saratoga Campus in Saratoga Springs, NY.

Job Highlights

• Monday – Friday 8:30am – 4:30 (or 9:00am – 5:00 pm)
• Case coordination, discharge planning, utilization management (managed care) issues
• Conduct group therapy sessions
• Engage family members in treatment by maintaining close communication and gathering data
• Interfacing with the outpatient/community providers, including school personnel
