Job summary

American Senior Communities, LLC is seeking a Clinical Registered Dietitian to support residents at Valparaiso Care and Rehab and Lowell Healthcare. The role focuses on assessing nutritional status and collaborating with the care team to develop individualized nutrition plans.

Travel between Valparaiso Care and Rehabilitation and Lowell Healthcare is required (3 days/week at Valparaiso, 1 day at Lowell) with remote eligibility after 90 days.

Qualifications

• Must have and maintain current dietitian registration.
• Current certification and/or license in the state(s) in which practicing.
• Nutrition documentation experience preferred.

Responsibilities

• Uses clinical knowledge to perform assessments to identify residents’ nutrition needs and those at nutritional risk.
• Interviews residents, family members, legal representatives, and significant others to obtain information for individualized plans of care.
• Works cooperatively with interdisciplinary and clinical teams.
• Conducts meal observations and interviews staff and residents to ensure appropriate food amount, type, and frequency to maintain nutrition parameters.
