Job summary

Benedictine is seeking a licensed Physical Therapist to assess and plan care for residents across BHS communities. The role requires current state licensure and a Bachelor's degree, with responsibilities including developing care plans, discharge planning, and education for families and staff.

The PT will provide therapeutic treatment across sites as needed, potentially traveling between locations within a day to meet resident needs.

Qualifications

• Current state licensure in Physical Therapy, in good standing.
• Bachelor's degree required.

Responsibilities

• Develops and revises plan of care based on the residents' needs.
• Plans for discharge and anticipates for long term and/or short term residents' needs.
• Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
• Provides treatment to all BHS communities as needed; may be required to travel to multiple sites in a day.
