Job summary

Benedictine is seeking a licensed Physical Therapist in Minnesota to evaluate residents, perform ongoing assessments, and plan and implement therapy interventions.

You will develop and revise care plans, establish functional goals, plan for discharge, and educate the care team, residents, and families on the care plan. This role requires current licensure in Physical Therapy and a Bachelor's degree. This is a hands-on role in a supportive, interdisciplinary team environment.

Qualifications

• Current state licensure in Physical Therapy, in good standing.
• Bachelor's degree.

Responsibilities

• Evaluates residents, conducts ongoing assessments, and plans and implements therapy interventions.
• Develops and revises plan of care based on residents' needs, establishes functional goals, outcomes, and discharge plans.
• Plans for residents' discharge and anticipates long term and/or short term needs.
• Educates other health care team members, residents, family, and caregivers on information regarding the care plan.
