This position includes performing comprehensive assessments, reviewing medical histories, and providing patient education. Applicants should have a Master's Degree, an active NP license, and a current BLS certification.

Qualifications

• Master’s Degree OR equivalent experience with NP license.
• Active State NP license and National board certification.
• Current BLS certification required.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure timely documentation.
• Conduct medical reconciliation for patient safety.
• Provide patient education on wellness.
