Job summary

Hueman is seeking Nurse Practitioners to conduct in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in Clovis, NM. This position allows you to customize your work schedule, promoting both professional growth and flexibility in your daily life.

Candidates will travel to patients’ homes, perform physical assessments, and review medical histories. A Master’s degree or equivalent experience, along with active licenses and certifications, are required.

Qualifications

• Master's degree or equivalent experience and NP license required.
• Active State NP license needed.
• Active national board certification is mandatory.
• Current BLS certification must be maintained.
• Preferred: 12 months experience as a paid advanced clinical provider.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure documentation accuracy.
• Conduct medical reconciliation for patient safety.
• Provide basic patient education on wellness.
