Job summary

Your Health Organization is seeking a Registered Nurse to serve patients across the Athens area as a Nurse Case Manager. The role involves home and facility visits, coordinating with providers, and initiating telemedicine when appropriate.

Travel is required daily; the position is full-time, with typical 8-hour shifts from 8am to 5pm, Monday through Friday. The service area includes Jasper, Ellijay, Blue Ridge and surrounding regions.

Qualifications

• Must be a licensed Registered Nurse in good standing.
• Minimum of three years clinical experience preferred.
• Experience in community/home health settings is a plus.
• Strong communication and collaboration with care teams and patients.

Responsibilities

• Facilitate receiving medical records from patient’s providers and specialists.
• Review medical records and enroll patients in Care Management.
• Initiate a Care Management Plan of Care for eligible patients.
• Coordinate care with the care team and document activities accurately.
• Evaluate for home health, RPM devices, and possible hospitalizations.
• Travel daily to patient homes and facilities as required.
