The position involves traveling through Franklin and Licking County to provide diagnostic assessments, therapy visits, and up-to-date treatment planning in clients' home or community settings.

Qualifications

• Master’s or higher in Social Work or Counseling required.
• Licensure as LSW/LISW/LISW-S/LPC/LPCC/LPCC-S required or eligible.
• Travel across Franklin and Licking County as part of the role.
• Driver’s license with reliable transportation required.

Responsibilities

• Performing comprehensive diagnostic assessments.
• Providing patient visits as scheduled.
• Ensuring treatment plans are up to date for each client.
• Completing comprehensive and succinct progress notes.
• Communicating and following up regarding client status.
• Working collaboratively with case management and psychiatric services.

Education

Master’s degree in Social Work or Professional Counseling

Bachelor’s degree in Social Work or related field
