Job summary

ViaQuest is seeking a Therapist (LSW/LPC/LMFT) for full- or part-time roles, with travel across Franklin and Licking County. Responsibilities include diagnostic assessments, patient visits, and updating treatment plans within a collaborative, community-based team.

Applicants should have a social work or counseling degree and relevant licensure (LSW/LISW/LISW-S/LPC/LPCC/LPCC-S). Flexible scheduling and travel are expected; a valid driver’s license is needed.

Qualifications

• Bachelor’s degree in Social Work or a related field with one year of therapy experience or a Master’s degree in Social Work or Professional Counseling.
• Social Work/Professional Counseling License or equivalent: LSW, LISW, LISW-S, LPC, LPCC, LPCC-S.
• Experience in therapy and counseling is preferred.
• Experience within a home setting is preferred.

Responsibilities

• Perform comprehensive diagnostic assessments.
• Provide patient visits as scheduled.
• Ensure treatment plans are up to date for each client.
• Complete comprehensive and succinct progress notes.
• Communicate and follow up regarding client status.
• Collaborate with case management and psychiatric services.
